Does NAD+ improve skin health? The honest, current answer is: possibly, but mostly indirectly — and mainly at the cellular level rather than through dermatology-specific human trials. NAD+ is a coenzyme skin cells rely on for energy production and DNA repair, and its natural decline with age has been linked, in laboratory and animal research, to reduced cellular resilience. Much of what circulates online about NAD+ and skin health is extrapolated from these preclinical studies, or borrowed from research on niacinamide — a related but different compound — rather than from trials of NAD+ injections themselves.

How NAD+ Skin Health Claims Start at the Cellular Level

Skin cells are metabolically demanding, and NAD+ sits at the centre of the reactions that keep them running. It's a required cofactor for PARP enzymes, which are involved in repairing DNA damage — including the kind of damage caused by UV exposure. NAD+ also feeds the mitochondrial pathways that generate cellular energy, and helps regulate the body's response to oxidative stress. Laboratory studies on skin cells (keratinocytes) have shown that restoring NAD+ levels supports these repair and survival pathways after UV exposure, and researchers have observed that NAD+ levels in skin tissue tend to fall with age, alongside changes in resilience that are commonly associated with older skin. What the Research Actually Shows — and Where It Stops

A 2025 review of NAD+ precursors in dermatology, published in Cosmoderma, is unusually candid about the strength of the evidence behind different delivery methods. Oral nicotinamide has moderate-quality evidence from randomised trials, mostly focused on photoprotection rather than cosmetic outcomes. Topical NAD+-related formulations have moderate-to-high evidence from actual dermatology studies. Intravenous or injectable NAD+, by contrast, has low-quality evidence — the review notes there are currently no dermatology-specific clinical trials of injectable NAD+ for skin outcomes. The authors conclude that most anti-ageing claims made about NAD+ are “investigational rather than established dermatologic therapies.” That's a similar conclusion to what we found when we looked at the broader ageing claims made about NAD+ — the mechanisms are real, but the leap to visible results in a mirror isn't yet backed by the kind of trials that would let anyone promise it.

Niacinamide Skincare Isn't the Same Claim as an NAD+ Injection

It's easy to see “NAD+” and “niacinamide” in the same sentence and assume they're interchangeable — they're not. Niacinamide (vitamin B3) is a precursor your body converts into NAD+, and it has a genuinely solid evidence base of its own when applied topically: clinical studies support its role in strengthening the skin barrier, evening out tone, and improving texture. That evidence describes niacinamide applied directly to skin, in specific concentrations, over weeks. It doesn't automatically transfer to NAD+ delivered by injection, which raises NAD+ levels systemically rather than at the skin surface. Practical Considerations If You're Curious About NAD+ and Skin

If skin health is your main interest, the best-evidenced levers are still the boring ones: daily sun protection, adequate sleep, and a topical routine with ingredients that have direct dermatology trial data behind them. NAD+ supplementation is generally discussed — including by us — in the context of broader cellular energy and healthy ageing support, not as a skincare product, and the current evidence doesn't support treating it as one.
